Ein 75 jahriger Patient stellte sich mit seit 2 Jahren langsam grosenprogredienten Raumforderungen unterhalb beider Schulterblatter vor. Anamnestisch bestand ein Zustand nach proximaler Oberarmamputation linksseitig mit folgender prothetischer Versorgung. Klinisch zeigten sich direkt unter den Schulterblattspitzen beidseits 2 faustgrose Tumoren, die fest mit dem tiefer gelegenen Gewebe verwachsen waren. Sowohl magnetresonanztomographisch als auch intraoperativ stellte sich ein infiltratives Wachstum dieser Raumforderungen in die seitliche Thoraxmuskulatur und das Periost der Rippen sowie des Schulterblatts dar, so das ein maligner Tumor vermutet wurde. Hyperalgesia and opioid switching

Opioids, intended to abolish pain, can unexpectedly produce hyperalgesia, particularly during rapid opioid escalation. Opioid switching could be a therapeutic option in a condition of opioid-induced tolerance or hyperalgesia, but conversion ratios between opioids are difficult to apply in this context and require strict surveillance and expertise. This situation is challenging, because the rapid escalation of opioid doses, possibly due to the development of opioid-induced tolerance, can cause hyperalgesia. Preoperative detection of intrathoracic tumor spread of esophageal cancer: endosonography versus computed tomography.

The results of endosonography and computed tomography, using an optimized CT technique, have been compared in the preoperative detection of intrathoracic tumor spread of esophageal cancer. In 22/40 patients with esophageal tumors complete passage of the ultrasonic endoscope was possible. Endosonography was superior to CT in the assessment of early stage of esophageal tumor (T1-2). Out of 9 tumors confined to the esophageal wall, 8 were classified correctly by endosonography and only 5 by computed tomography. Metastatic seeding of colon adenocarcinoma manifesting as synchronous breast and chest wall localization: report of a case.

Colon carcinoma rarely metastasizes to the breast and it is usually associated with a poor prognosis. Even rarer is metastatic seeding of colon cancer cells in an intramammary location after surgery. Including a primary breast malignancy in the differential diagnosis of such cases is mandatory. We report a rare case of double seeding implantation of colon adenocarcinoma inside the breast parenchyma and intercostal muscles 6 years after resection of a pulmonary metastasis from colon adenocarcinoma.
